Join our team as a Social Worker where you can make a vital difference in our residents’ lives!

Brevillier Village is an award winning, non-profit housing and health care community for seniors. Not only are we conveniently located in Harborcreek Township—just 15 minutes from downtown Erie, PA— but our campus stands on over 20 acres of picturesque lakefront property.

Classification: Full Time, non-exempt; 40 hrs./week and one Saturday/Sunday per month

Social Worker Requirements:

• Possess a bachelor’s degree in the field of Social Services.
• Two years’ experience in long term care/personal care home setting preferred.
• Have a working knowledge of Federal and State regulations, and acceptance of Brevillier Village policy of nondiscrimination and Personnel Policies.
• Ability to organize work and follow through on assigned tasks in a timely manner.
• Good written and verbal communication skills.
• Shows proper respect and understanding when caring for the elderly.
• Experience with Dementia patients/residents
• Must be able to sit, stand, walk intermittently, lift and/or maneuver a minimum of 25 pounds.

POSITION INVOLVES BUT NOT LIMITED TO:

Providing direct services to residents and families of Brevillier Village including:

1. Ongoing individual casework in Brevillier Village as needed.
2. Group work with families and residents.
3. Program Development.
4. Coordination of resident association in Barnabas Court North.
5. Assisting in tours of all projects.
6. Development of Resident Assessments and Support Plans; care plan meetings with residents and families.
7. Coordinating discharge planning for residents.
8. Resident and family advocate.
9. Documentation as specified by state and federal guidelines in the resident medical record.
