Community Resource Liaison

Job Summary:

A Community Resource Liaison supports students’ social determinants of health by coordinating referrals, case management, and access to essential services such as food, housing stability, transportation, healthcare navigation, and other community resources. The Liaison works closely with families, school staff, and community partners to identify barriers to learning and connect students with supports that promote overall well‑being and stability. Position reflects School District calendar work days.

Essential Functions of the Position:

• Complete intakes for youth referred.
• Maintain individual client charts.
• Case Management and linkage to services or supports in the community.
• Work with educators and school professionals and provide therapeutic support.
• Participate in school and community events.

Required Education, Experience, and Clearances:

• Master’s degree in social service-related field preferred, Bachelor’s required.
• Knowledge of CASSP & special education principles.
• Experience working in the mental health field is required, preferably with children and families.
• FBI, Child Abuse History, PA State Police, and Driver Clearances.
