CASE MANAGER

Exempt

RESPONSIBLE TO:

The Case Manager is responsible to the Director of Client Services and Executive Director.

QUALIFICATIONS:

1. Bachelor’s degree in social work, psychology, related field, or equivalent experience.

2. Practical experience in human services, case management, and the ability to work cooperatively in a network of services for the homeless.

3. Ability to communicate clearly, both orally and in writing, and experience as a team member.

4. Ability to make decisions and maintain client confidentiality.

5. Commitment to women and an understanding of women's issues, including but not limited to Homelessness, Trauma Informed Care, Mental Disorders, and Substance/Alcohol Use Disorder.

6. Maturity, good judgment, and an ability to handle crisis situations.

NATURE AND SCOPE:

The Case Manager is responsible for coordinating with each resident her individual action plan of personal goals, so she can progress towards self-sufficiency and eventually affordable housing. The Case Manager is in regular contact with residents, MCW staff, caseworkers of social service agencies, and others involved with MCW clients. The Case Manager works under minimum supervision, within MCW policy and procedure guidelines, consulting with the Director of Client Services regarding special concerns and difficult situations.

The position has major challenges in the following areas:

Conveying a welcoming, helpful response to those seeking assistance and residence from Mercy Center for Women.

Serving as the direct link with Mercy Center for Women residents to assist them in securing the resources they need for themselves and their children.

Promoting the mission of Mercy Center for Women and expanding programs to meet the needs of all clients.

PRINCIPAL ACCOUNTABILITIES:

1. Counsel and direct the individual client in developing and implementing an action plan of personal goal setting. Help the client to secure community resources and obtain appropriate educational/ job training. Assist clients with applying for safe, affordable, permanent housing while at Mercy Center for Women.

2. Provide supportive listening to identify problems or issues and possible solutions. Provide accurate information, referral, and follow-up, as necessary.

3. Maintain awareness regarding current Mercy Center for Women programs and services in Erie County so as to give appropriate referral information.

4. Accompany clients to court hearings or other proceedings as deemed appropriate and provide transportation when necessary.

5. Communicate with other staff members to convey information in a timely manner concerning client and other house matters to assure continuity of services.

6. Provide client with referrals to follow-up services for the enhancement of their own self-development and living situation.

7. Share in the 24-hour "On Call" schedule of Mercy Center for Women.

8. Complete and maintain client records in compliance with data collection requirements and confidentiality regulations.

9. Serve as a member of the Mercy Center for Women team committed to fulfilling the mission of Mercy Center and maintain a professional attitude at all times.

10. Perform other duties that may be requested by the Director of Client Services & Executive Director.

11. Be available to assist with the day to day needs of the center as needed.

Pay: $24.00 per hour
